We see ourselves as your medical home, a place that manages all aspects of your healthcare. We use the latest technology, including electronic medical records, to communicate with each other and seamlessly coordinate your care. You can always communicate with your Cold Hollow team and access your health records using our secure Patient Portal.
Your primary care provider leads your care team, which includes your doctor or nurse practitioner, nurse or medical assistant, care coordinator, scheduler and medical biller. Our team practices “coaching” to help you get healthy, stay healthy and receive all the care and services that are right for you.
Continuity of care is extremely important to us. Let’s say that your chosen primary care provider is out of the office. Since your whole team knows you, we always have someone who can see you in case you have urgent health needs. We also offer you the convenience of same-day access. If you think you need to be seen today, we will typically find a place for you during our regular office hours.
We also believe in shared decision-making and self-management support. In other words, we explain your options and help you make decisions and set goals for yourself. We also provide coordinated care. That means we will help you find specialists, make referrals, arrange appointments and make sure they have all the information they need to care for you. If you must visit an ER or if you or a family member is admitted to a hospital, we will make sure we have all the necessary information to ensure that you receive recommended after-care. We will share our electronic medical records with you and your other providers to help prevent misunderstandings or medical errors.

At Cold Hollow Family Practice, we coordinate your care even if you are hospitalized. All too often, there is little coordination or inadequate communication between providers such as doctors, nurses, physical therapists and occupational therapists. We make sure that every member of your care team receives actionable medical information that is complete, relevant, appropriate and accurate. Discharge instructions that are given to patients following hospitalization may be lost, discarded or difficult to understand. We solve these problems by coordinating care and communications between all parties who are involved with your care.
